Patient Care Coordinator The Patient Care Coordinator is responsible for coordinating patient care services and ensuring that patients receive the highest quality of care possible.
The ideal candidate will have excellent communication skills, be detail-oriented, and have a passion for helping others.
Responsibilities:
Coordinate patient care services, including scheduling appointments, arranging transportation, and coordinating with healthcare providers Ensure that patients receive the highest quality of care possible Communicate with patients and their families to provide information and support Assist with patient education and provide resources to help patients manage their health Collaborate with healthcare providers to develop and implement care plans Document patient information and maintain accurate records Ensure compliance with all healthcare regulations and standards Requirements:
High school diploma or equivalent Previous experience in healthcare or customer service Excellent communication and interpersonal skills Strong organizational and time management skills Ability to work independently and as part of a team Proficient in Microsoft Office and electronic medical records systems Knowledge of healthcare regulations and standards.
Estimated Salary: $20 to $28 per hour based on qualifications.
